GENERAL DESCRIPTION

The Social Worker is responsible for interventions with policyholders and their families, including performing a social assessment, planning, implementing, and evaluating the social needs of policyholders with chronic and life‑threatening conditions. The Social Worker coordinates community‑based non‑clinical services necessary to maintain an optimal level of care to meet the biopsychosocial needs of high‑risk policyholders, considering their preferences, and plans daily work activities to efficiently complete assessments, coordination, and follow‑up within established timelines.

ESSENTIAL FUNCTIONS
  • Conducts assessments of policyholder's social history, considering the needs identified by clinical staff and Clinical Affairs delegates, either via telephone or at the policyholder's residence (including Vieques and Culebra). Develops an Individualized Care Plan focused on the biopsychosocial needs identified through the assessment.
  • Performs telephone interventions with policyholders and families to identify social, emotional, and support needs; provides guidance on appropriate services and documents interventions in electronic files.
  • Maintains appropriate interaction with the policyholder, caregivers, family, and providers and integrates them into the care plan as an integral part of the program’s interdisciplinary team.
  • Coordinates referrals with internal departments, external agencies, and community groups to address policyholders’ social and emotional needs.
  • Guides policyholders, guardians, or authorized representatives and their families on social aspects, including rights and responsibilities, as needed.
  • Maintains interaction and promotes open lines of communication with external and community agencies to coordinate the necessary services.
  • Participates in the evaluation process of cases referred by other units via telephone or at the policyholder’s residence.
  • Serves as a consultant and educator for the unit staff regarding social and behavioral management issues.
  • Handles inquiries of member situations considered socially emergent and monitors the development of interventions in conjunction with the referral sources within the Clinical Affairs Department.
  • Develops activities to follow up on cases referred for social emergencies carried out by the clinical and non‑clinical units of the company.
  • Performs and maintains an updated record of the referred cases and documentation of actions taken in the different relevant electronic tools, including electronic files of members as requested.
  • Participates in department interdisciplinary meetings assigned as a Social Worker. Gathers information from the assigned tasks, performs the required interventions, and provides necessary and timely feedback.
  • Must comply fully and consistently with all company policies and procedures, local and federal laws, and the regulations applicable to our industry to maintain appropriate business and employment practices.
  • May carry out other duties and responsibilities as assigned, according to the requirements of education and experience contained in this document.
MINIMUM QUALIFICATIONS

Education and Experience: Master’s degree in Social Work. At least five (5) years of experience practicing social work.

Certifications / Licenses: License and Registration to practice Social Work in PR is required. A valid driver’s license in the Commonwealth of Puerto Rico is required. Other: Experience in palliative care, preferably. Must own a car in good condition. Availability to travel throughout the island according to the cases.
