Now Hiring: Social Worker

Shift: Full Time, 8hrs, onsite

We are seeking a compassionate and dedicated Social Worker to join our team at Champion City Nursing & Rehabilitation in Pittsburgh, Pennsylvania. As a Social Worker, you will play a vital role in providing emotional and psychological support to our residents, addressing their needs and concerns, and helping them maintain their independence and well-being.

Responsibilities:

• Assess the social, emotional, and psychological needs of residents and develop personalized care plans to address their concerns and improve their overall quality of life
• Provide counseling, guidance, and support to residents and their families to help them cope with the challenges of aging or illness
• Facilitate communication between residents, families, and healthcare teams to ensure that residents receive holistic care and support
