Job summary

Pardee UNC Health Care seeks an Inpatient Social Worker to provide ongoing care management and comprehensive assessment, planning, implementation and evaluation of patient needs. The role focuses on improving patient management, satisfaction and continuity of care within an acute hospital setting.

The position requires strong organization, adaptability to change and compliance with regulatory guidelines. Day-shift, onsite work with a 40-hour week is typical for this role.

Qualifications

• Bachelor's degree in Social Work (BSW) from a CSWE-accredited program.
• BLS Certification is required.

Responsibilities

• Assesses and documents bio-psychosocial needs and prepares an intervention plan.
• Evaluates patient/family crises, provides crisis intervention and creates plans for resolution.
• Delivers information and short-term therapeutic counseling to patients and families on social, financial, and family matters.
• Develops and maintains relationships with community health and social agencies; monitors resources.
• Refers patients and families to appropriate community assistance resources.
• Obtains and documents information about patients and families for communication with the care team.

Summary

The Social Worker provides ongoing care management support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of care management, utilization review and discharge planning. The Inpatient Care Management Social Worker must be highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

Responsibilities

• Assesses and documents bio-psychosocial needs and prepares a formal intervention plan to suit.


• Evaluates and identifies possible or actual patient/family crises, providing crisis intervention and emotional support, and creating and facilitating effective plans for resolution.


• Delivers information and short-term therapeutic counseling to patients and their families in relation to social, psychological, financial, and family situations.


• Develops and maintains relationships with outside community health and social agencies. Monitors ongoing and new community resources for meeting patient needs.


• Refers patients and families with appropriate community assistance resources.


• Obtains, interprets and documents necessary information regarding patients and families, including bio-psychosocial history factors, and communicates to interdisciplinary care team.
