Job summary

CivicMinds, Inc. is seeking a licensed clinical social worker (LCSW) in New York to provide psychotherapy, psychosocial assessments, and coordinated care within a collaborative team.

The hybrid role emphasizes evidence-based treatment planning, crisis assessment, and referrals to community resources. An MSW and active NY LCSW license are required, with 1–2 years of clinical experience encouraged. Bilingual English/Spanish is a plus.

Responsibilities

• Conduct comprehensive psychosocial and mental health assessments.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual and group counseling services.
• Perform crisis assessments, triage, and appropriate interventions.
• Track client progress and adjust treatment plans as needed.
• Coordinate care and make referrals to healthcare providers and community resources.
• Maintain accurate, timely, and confidential clinical documentation.
• Collaborate with interdisciplinary teams to provide integrated healthcare services.
• Support clients' overall mental health and wellness through ongoing care and education.
