Job description

Responsibilities
  • Assist members in person or by phone with challenges such as depression, anxiety, substance use, mental health concerns, family and child-related issues, domestic violence, and job jeopardy. Provide assessments, referrals, and follow-up support.
  • Offer ongoing support and interventions for members admitted for psychiatric care, detoxification, and rehabilitation.
  • Collaborate with families, educators, therapists, and community partners to ensure cohesive care for clients.
  • Provide members with personalized referrals to community-based resources and behavioral healthcare providers.
  • Utilize QNXT, Salesforce, DMS, ICIS, V3, and other databases, as needed, to complete tasks, authorizations, and documentation.
  • Document services and progress in accordance with clinical and regulatory standards.
  • Review and Process Behavioral Health Prior Authorizations, including for Intensive Outpatient Programs (IOP) Partial Hospitalization Programs (PHP), Transcranial Magnetic Stimulation (TMS), and Applied Behavior Analysis (ABA) prior-authorizations.
  • Provide emotional well-being workshops, grief support, and crisis support, both in the field and virtually.
  • Participate in health fairs and tabling events. Provide information on healthy lifestyles and encourage members to participate in healthy living programs offered through Wellness/MAP or vendor programs such as Weight Watchers, Noom, Flytehealth, and Virta, etc...
  • Participate in member-support programs, such as Post-ER Outreach, Maternal Health Program, Adolescent Crisis Intervention Program, and other initiatives as needed.
  • Collaborate with MAP/Wellness nurses, health educators, and nutritionists.
  • Perform additional duties as assigned by management.
Qualifications
  • Master’s degree in social work or equivalent years of experience required; New York State License (LMSW) required.
  • Minimum two (2) years post-graduate experience in direct social work required; Autism and ABA therapy experience highly preferred.
  • Bilingual in Spanish preferred.
  • Excellent written, listening, verbal communication, and interpersonal skills.
  • Demonstrate sound judgment and discretion in establishing and maintaining effective work habits and relationships, while ensuring confidentiality in all interactions.
  • Ability to work well under pressure, maintain composure, multi-task, establish priorities, meet deadlines, and follow through on assignments.
  • Strong writing, listening, verbal communication, and interpersonal skills.
  • Ability to work flexible hours, including some evenings and weekends.
  • Ability to travel independently to locations across the five boroughs of NYC for field events
