Job summary

TBH is seeking a Social Worker to deliver comprehensive social work services within a hospital setting. The role includes care coordination, planning, counseling, entitlements, and connecting patients with community resources to support chronic disease management.

You will provide consultation and education to clinicians, and deliver services in the hospital or community, adhering to NASW and Case Management Standards in alignment with the hospital's mission and goals.

Qualifications

• Hospital work experience is required.
• New York LMSW license is required.
• Screen certification is required for inpatient social workers (90-day window for new hires).
• Broad knowledge of community resources and social work/case management regulatory requirements.
• Knowledge of computer applications (word processing, spreadsheets, clinical software) preferred.
• Knowledge of evidence-based therapeutic techniques.
• Ability to handle high-stress situations.

Responsibilities

• Deliver social work services including care coordination, planning, counseling, entitlements and connecting patients to community resources for chronic disease management.
• Provide consultation, education and other services as requested by clinicians within the department, in the hospital or in the community.
• Provide social work services in accordance with NASW, Case Management Standards and the hospital's mission and goals.
• Coordinate with care teams to support discharge planning and follow-up care.

• New York State License Masters Social Worker (LMSW)
• Screen certification -- Required for Social Workers in In-Patient Units. Effective 1/1/2023, new hires will have 90 days to obtain.

Job Description

The Social Worker is accountable for the delivery of social work services including care coordination, planning, concrete and support counseling, entitlements and other community resources regarding chronic disease management. Provides consultation, education, and other services as requested by the clinicians within the Department, in the Hospital or in the community. Provides social work services in accordance with National Association of Social Worker (NASW), Case Management Standards and the Hospital's mission and goals.
