Job summary

Yale NewHaven Health seeks a Social Worker to assess, intervene, and follow up on referred patients, providing continuum care planning and advocacy. This role also acts as a liaison between patients and community service systems, serving patients of all ages and backgrounds.

Responsibilities emphasize ethics, patient-centered care, and collaboration with interdisciplinary teams to support care and access to resources within the community.

Qualifications

  • Bachelor of Social Work degree from a CSWE-accredited program or equivalent bachelor's degree with experience in a related helping profession or with community agencies.

Qualifications

EDUCATION


Bachelor of Social Work degree from a program accredited by the Council on Social Work Education or equivalent bachelor's degree with experience in working in a related helping profession or with community agencies.


EXPERIENCE


May be a new graduate. Preference is given if experienced in health or mental health social work or related helping profession.


SPECIAL SKILLS


Competency in caring for patients treated at the Hospital regardless of age, disability, sexual preference, gender, ethnicity, or diagnosis. Computer literacy.
