Job summary

Qualifications

• 2 years related experience with at least 1 year in a health care setting (hospital, clinic, rehabilitation center, etc.).
• Bachelor's Degree in Social Work, Psychology, Gerontology or Sociology from an accredited institution is required.
• Excellent problem solving, customer-focused approach, strong communication and listening skills, organized, and awareness of nonverbal communication. Proficiency in MS Office.

Schedule: 8-5 / Monday - Friday

We are seeking a compassionate, resident-focused Social Worker to join our Skilled Nursing Facility team. In this role, you will advocate for residents, support families, and help ensure the highest quality of care and quality of life for those we serve.

Position Summary

The Social Worker is responsible for assessing residents' psychosocial needs, coordinating services, facilitating care planning, and providing emotional support to residents and families throughout their stay. This position works collaboratively with the interdisciplinary team to promote resident well-being, successful transitions of care, and compliance with regulatory requirements.

Qualifications

Qualified applicants are required to possess 2 years related experience with at least 1 year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.).

Bachelor's Degree in Social Work, Psychology, Gerontology or Sociology from an accredited institution is required.

Additional requirements include excellent problem solving skills, using a customer-focused approach to problem solving and goal setting, excellent communication and listening skills, enthusiastic, resourceful, organized and possesses strong personal awareness of nonverbal communication. Computer skills including proficiency in MS Office.
