General Summary:
Under the supervision of the Personal Health Director, provides specialized counseling to assist clients with emotional, social, financial and environmental problems which inhibit the health of themselves and their family. Services may be provided in preventive health, home health and clinic programs and include assessment, teaching, counseling and prevention and referral services to individuals, families and groups to promote health and wellness to clients in the county. Maintains patient records and program activity records and prepares required reports.
Employment Qualifications:
Education: Possession of a Bachelor’s Degree in Social Work (BSW). Possession of a Master’s Degree in Social Work (MSW) preferred.
Experience: One year of Social work experience dealing with families.
Licenses & Certifications:Possession of a current Michigan license in Social Work.Possession of a valid driver's license.
