TITLE:Social Worker-- Primary Care Clinic
DEPARTMENT:Behavioral Health/Social Services
REPORTS TO:Behavioral Health/Social Services Manager
POSITION STATUS/HOURS:Full-time .9 FTE (36 hours/week). Monday through Friday- with hours scheduled 5 days per week. Scheduled hours will include starting at 8a and ending at 3:30p with the exception of one day a week ending at 4p.
JOB SUMMARY:
The Social Worker works with patients to coordinate community programs and clinical service needs. The Social Worker works closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
The social worker will spend the majority of time in the Primary Care Clinic in Lancaster, with flexibility to travel to outreach clinics when needed (Fennimore, Cassville and Potosi). The social worker will also provide regular social work coverage on Med/Surg for social work needs and discharge planning. The social worker will be available to assist in the Emergency Department, Specialty Clinics, Outpatient/Rehabilitation Departments, OB, and other areas of the hospital to provide social work assistance and discharge planning upon request.
SOCIAL WORKER JOB DUTIES:
QUALIFICATIONS:
1.Bachelor’s Degree in social work or related field with a Wisconsin Certified Social Worker (CSW) license, in good standing, required.
2.Minimum of 1-year social work experience in a health care setting, clinic setting or casement experience.
3.Experience with EPIC medical record system is beneficial but not required.
