Job summary

Qualifications

• Active LISW or LISW-S license in the state of practice.
• MSW from an accredited program.
• Healthcare or medical social work experience preferred.
• Strong clinical judgment, communication, and ethical patient-centered care.
• Knowledge of social determinants of health and discharge planning.

Responsibilities

• Provide psychosocial assessments, counseling, and care planning for patients and families.
• Coordinate discharge planning, community referrals, and resource linkage.
• Collaborate with physicians, nurses, and researchers to optimize patient outcomes.
• Maintain accurate clinical documentation per legal and ethical standards.
• Advocate for patients addressing social determinants of health.
• Participate in quality improvement initiatives, case conferences, and program development.
• Provide crisis intervention, risk assessments, and safety planning.
• Educate patients and families on treatment options and support services.
• Knowledge of trauma-informed care and motivational interviewing in practice.
