Cynet HealthStaff in Juneau, Alaska is seeking a Social Worker to provide evidence-based outpatient behavioral health treatment for individuals with behavioral, emotional, and mental disorders. Responsibilities include individual, group, family, and marital therapy, as well as assessments, care coordination, and crisis intervention.

• Master’s Degree in Social Work or related field from an accredited institution.
• Licensed or eligible for licensure as LCSW/LPC/LMFT preferred.
• Valid state driver’s license or ability to obtain within 6 months of hire.
• Clinicians who are not yet licensed will receive clinical supervision.
• BLS within 3 months; Clinical Competency Assessment at 3 months and every 3 years.

Responsibilities

• Implement culturally relevant screening, assessment, diagnosing, treatment planning, and evidence-based individual, group, and family therapy.
• Provide care coordination, advocacy, motivational interviewing, bio-psychosocial education.
• Maintain complete documentation in the electronic health record with timely notes and plans.
• Provide crisis intervention services within normal clinic hours and after-hours on a shared rotating basis.

Key Essential Functions and Accountabilities of the Job:Implement culturally relevant screening, assessment, diagnosing, treatment planning, and evidence-based individual, group, and family therapy to meet client needs.Utilize Trauma Informed Care in all aspects of patient, employee, and community interactions.Integrate mental health and substance use disorder interventions based on individual assessments and treatment planning.Develop individualized treatment plans and treatment reviews based upon individual needs.Provide care coordination, advocacy, motivational interviewing, bio-psychosocial education, and skill development.Maximize access to care and effective utilization of services by improving coping strategies and skills, promoting self-care, improving self-understanding, and managing symptoms.Communicate and collaborate with clinical and paraprofessional team members to achieve individualized treatment goals.Provide crisis intervention services within normal clinic hours.In rural clinics, provide shared rotating after-hours crisis and emergency services.Maintain complete, timely, and accurate documentation of all services within the electronic health record including assessments, progress notes, treatment plans, screening tools, and discharge plans as required by policy and procedure.Self-monitor and fulfill licensure requirements as mandated by the clinician’s licensing board.Adhere to patient confidentiality, sharing, and release of information per HIPAA and policy.Coordinate care within the health system and local community social agencies to ensure appropriate referrals.Attend, engage, and professionally participate in meetings and training.Consistently meet productivity expectations to support ongoing patient access to timely care.Promptly respond to coding and audit requests and corrections in a respectful, professional, and timely manner.Complete all required provider enrollment expectations and documentation.Other duties as assigned.

Education and Experience Requirements:Experience with behavioral health assessment and diagnoses is required.Ability to provide assessment, treatment plans, and referral for patients of all ages.Knowledge of behavioral health assessment and diagnostic evaluation criteria.Experience in treatment plan development and patient/family-centered goals and objectives.Familiarity with evidence-based methods used in individual, group, and family therapy for emotional, behavioral, and mental health issues.Understanding of federal and state mandatory reporting requirements, ethical and legal management practices, patient rights, and documentation regulations specific to federal and state regulations.

Skills and Abilities:Ability to assess patients for risk of suicide or self-harm and develop effective safety plans.Advocacy skills to coordinate required services or resolve problems in crisis situations.Analytical skills to evaluate, diagnose situations, and adopt effective courses of action.Ability to document clear and concise reports based on direct patient observations in compliance with professional standards.Flexibility to modify treatment activities or approaches as necessary to meet changes in patients
