DUTIES:
Social work evaluation of patient/family psych/social and environmental needs.
Implements the social work treatment plan and documents consistent with insurance guidelines in their homes or facilities
Assists the primary nurse with establishing and maintaining a treatment plan for each patient/family.
Provides the psychosocial needs of the hospice patient/family.
Assesses the needs of the bereaved client through verbal and face-to-face interviews.
Provides end-of-life bereavement supportive services for patients and their families.
Understands and discusses available community resources with the client for possible referrals.
Participates in community education on grief and bereavement issues.
Provides support to selected patients post discharge.

POSITION QUALIFICATIONS:
