Adventist Health Glendale is seeking a Social Worker Care Management to coordinate patient discharge and post-hospital care plans in Glendale, California. The role requires an MSW and acute care experience, with day shift hours 8:00 AM–4:30 PM.

You will provide counseling and emotional support to patients and families, assess psychosocial barriers to discharge, and work closely with healthcare teams, families, and outside agencies to ensure safe and smooth transitions.
