Methodist Fremont Health-Dunklau Gardens is looking for a full-time Social Worker to join our team. This role focuses on discharge planning and support for complex and at-risk patients, including long-term care residents. It involves assessing and addressing psychosocial, emotional, and financial needs; facilitating eligibility for social programs; coordinating with community resources; and providing ongoing counseling and advocacy to ensure quality outcomes and an improved patient/resident experience throughout their stay.

Job Duties and Responsibilities:

  • Develops and implements complex discharge plans by coordinating with patients, families, and post-acute agencies, addressing barriers early, and ensuring continuity of care based on patient needs and available resources.
  • Provides support for legal and end-of-life planning, including assistance with advance directives, power of attorney, and facilitating discussions around goals of care.
  • Collaborates with interdisciplinary teams through participation in rounds, family meetings, and care planning for patients with complex medical, behavioral, and psychosocial needs.
  • Advocates for diverse and high-risk patient populations, offering counseling, financial assistance (e.g., Medicaid), and support for patients from all cultural and socioeconomic backgrounds.
  • Maintains thorough documentation and compliance, communicates effectively using AIDET principles, and contributes to hospital and community initiatives that enhance patient care and safety.

  • Monday - Friday 8:00 a.m. to 4:30 p.m.

Job Requirements:

  • A Bachelor’s degree in social work. May consider Bachelor’s degree in public health or social services required.
  • Master’s degree e.g., Master of Arts (MA), Master of Science (MS), Master of Business Administration (MBA), Master of Science in Nursing (MSN), Master in Social Work (MSW) or equivalent (e.g., Care Programme Approach [CPA], Practice Educator [PE], etc.) preferred.
  • Master of Social Work (MSW) degree from a school accredited by the Council on Social Work Education preferred.

Experience

  • 0-1 years of experience as a social worker or like experience working with the geriatric population required.
  • 2-3 years of experience as a medical social worker preferred.
  • Experience working with the geriatric population within the healthcare environment preferred.

License/Certifications:

  • Certified Social Worker (CSW) required within 4 years of hire.
  • Certified Master Social Work (CMSW) preferred.
