Job summary

Wedgewood Garden Care Center in Freehold, NJ is seeking a Licensed Clinical Social Worker (LCSW) to support residents and families with psychosocial needs, develop care plans, and advocate for access to community resources.

You will collaborate with an interdisciplinary team, maintain electronic records, and ensure high-quality, person-centered care for seniors, while pursuing Medicare/Medicaid credentialing as needed.

Qualifications

• Active LCSW license in New Jersey is required.
• Compassionate approach with seniors and families.
• Strong organizational and communication abilities.
• Medicare/Medicaid credentialing a plus or willingness to obtain.
