POSITION SUMMARY
The social worker is the member of the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Participates in the preparation, evaluation, and implementation of the interdisciplinary plan of care.
- Assures that the highest quality of social services is offered to patients/families and that the goals.of continuity and comprehensiveness of care are achieved.
- Provides psychosocial and emotional support to the patients/families.
- Maintains appropriate documentation in the hospice patient chart.
- Attends Interdisciplinary Team meetings for case review and development of plan of care,
- Participates in in-service education programs.
- Facilitates appropriate community resources and provides casework.
- Act as a resource for other interdisciplinary team members of the interdisciplinary team at the direction of the team manager.
- Completes assessment timely per policy.
- Accepts and performs other related duties and responsibilities as required.
SUPERVISORY RESPONSIBILITIES
- The incumbent in this position will have no direct reports.
EDUCATION/EXPERIENCE/CREDENTIALS
- At least 18 years of age.
- Master of Social Worker Degree (MSW) from a school accredited by the Council on Social Work Education REQURED
- and clinical experience relevant to the psychosocial needs of patients and families.
- One (1) year of social work experience in a health care setting.
- Current driver's license and proof of insurance required.
- BLS required. Travel required.
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of stress of repeated loss as well as to provide support to the rest of the team.
- Knowledge of community resources and the ability to utilize these resources.
- Demonstrates the ability to use good listening and communication skills.
- Able to multi-task effectively.
- Capacity to relate and work as a member of an interdisciplinary team.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

- Hearing / Visual Acuity for adequate for use with computers or telephone approximately 50% of day

- Hand grip dexterity approximately 40% of day
- Computer Highly technical work environment - Must be able to work >5 hours/day using keyboard, mouse, and monitor.
