Job description
POSITION SUMMARY

The social worker is the member of the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Participates in the preparation, evaluation, and implementation of the interdisciplinary plan of care.
  • Assures that the highest quality of social services is offered to patients/families and that the goals.of continuity and comprehensiveness of care are achieved.
  • Provides psychosocial and emotional support to the patients/families.
  • Maintains appropriate documentation in the hospice patient chart.
  • Attends Interdisciplinary Team meetings for case review and development of plan of care,
  • Participates in in-service education programs.
  • Facilitates appropriate community resources and provides casework.
  • Act as a resource for other interdisciplinary team members of the interdisciplinary team at the direction of the team manager.
  • Completes assessment timely per policy.
  • Accepts and performs other related duties and responsibilities as required.
SUPERVISORY RESPONSIBILITIES
  • The incumbent in this position will have no direct reports.
EDUCATION/EXPERIENCE/CREDENTIALS
  • At least 18 years of age.
  • Master of Social Worker Degree (MSW) from a school accredited by the Council on Social Work Education REQURED
  • and clinical experience relevant to the psychosocial needs of patients and families.
  • One (1) year of social work experience in a health care setting.
  • Current driver's license and proof of insurance required.
  • BLS required. Travel required.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Knowledge of stress of repeated loss as well as to provide support to the rest of the team.
  • Knowledge of community resources and the ability to utilize these resources.
  • Demonstrates the ability to use good listening and communication skills.
  • Able to multi-task effectively.
  • Capacity to relate and work as a member of an interdisciplinary team.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
PHYSICAL REQUIREMENTS
  • Sitting approximately 60% of day
  • Standing approximately 20% of day
  • Walking approximately 20% of day
  • Lifting 0 - 40 lbs approximately 20% of day
  • Bending approximately 10% of day. Kneeling
  • Hearing / Visual Acuity for adequate for use with computers or telephone approximately 50% of day
  • Reaching above head 75 degrees approximately 25% of day.
  • Hand grip dexterity approximately 40% of day
  • Computer Highly technical work environment - Must be able to work >5 hours/day using keyboard, mouse, and monitor.
