Job summary

Mont Marie Rehabilitation and Healthcare Center is seeking a Social Worker for a full‑time role. You will assess residents’ psychosocial needs, develop individualized care plans, and coordinate services to support well‑being and quality of life.

Under Marquis Health Consulting Services guidance, you’ll collaborate with families and an interdisciplinary team, provide counseling, and ensure compliance with facility policies and regulations.

Qualifications

• Must be able to work full‑time hours and maintain a flexible schedule.
• Bachelor’s degree required; master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare‑related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast‑paced, team‑oriented healthcare environment.

• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.
