Our client, a healthcare organization dedicated to compassionate hospice care, is seeking a Social Worker to join their team. The role supports interdisciplinary team members, patients, and their families within the Clinical Operations department.

The candidate should have a BSW (MSW preferred), Michigan social work license, and at least one year of paid social work experience, preferably in hospice or healthcare settings. Excellent communication and confidentiality are essential.

Qualifications

• BSW degree required; MSW preferred.
• Michigan license to practice social work.
• Minimum of 1 year of paid social work experience, hospice/healthcare preferred.
• Strong communication skills and ability to maintain confidentiality.
• Proficiency with clinical documentation systems.

Responsibilities

• Collaborate with the interdisciplinary team to provide psychosocial assessments for hospice patients and families.
• Assist patients and families with placement, legal planning, funeral arrangements, and connecting with community resources.
• Evaluate social, emotional, and financial factors and develop tailored care plans.
• Offer counseling and grief support, respecting cultural diversity and individual needs.
• Document patient interactions accurately, ensuring compliance with organizational and regulatory standards.

Pay Range: $59,055 - $73,842

Shift: Monday - Friday, 8:00 AM - 5:00 PM with flexible scheduling; on-call rotation with additional compensation
