Job summary

Hearth Hospice is seeking a dedicated Licensed Master Social Worker to provide compassionate social work services for hospice patients and families in the Brunswick area. You will counsel and support emotional well-being, collaborating with an interdisciplinary team to address psychosocial needs.

The role requires an MSW and state license, a valid driver’s license, and at least 1 year of healthcare or hospice social work experience.

Qualifications

• MSW from an accredited school of social work is required.
• Licensed Social Worker in the state board is required.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Evaluate medical needs, home situations, financial resources, and community support.
• Develop individualized social work plans and collaborate with Bereavement Coordinator.
• Contribute to care plans and participate in interdisciplinary meetings.
• Provide empathetic counseling to patients and families.

Are you a dedicated Licensed Master Social Worker (LMSW) who believes compassionate care can change lives? At Hearth Hospice, we’re looking for a patient-centered professional to join our team.

As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times.

Schedule: Monday - Friday 40hrs weekly

Coverage Area: Brunswick area

Responsibilities

• Be a Supportive Guide: Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Connect Hearts and Homes: Evaluate medical needs, home situations, financial resources, and community support.
• Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator.

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements: Current unencumberedlicense as a Social Worker from the State Board of Social Work

$30.00 - $33.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
