Job summary

Bradford Hills in Bradford, PA seeks a Social Services Assistant to support the Social Services Director in identifying and addressing residents' social, emotional, and psychological needs, and to help develop each resident's full potential during their stay.

You will assist in discharge planning, organize family meetings, document care plans and progress notes, and support residents and families through transitions, while ensuring rights and comfort throughout the process.

Responsibilities

• Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment.
• Assist in collecting social history data and in the development of a comprehensive discharge plan, including the MDS and Care Plan for new admits, as directed by the Social Services Director.
• Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes.
• Assist families with the engraving and/or marking of resident’s personal items.
• Review resident’s rights with residents and staff and attend to various personal needs of the residents.
• Assist in facilitating family group meetings.
• Document progress notes, which relate to each resident’s care plan when necessary and within policy timeframe.
• Other special projects and duties, as assigned.
