Job summary

The U.S. Department of Veterans Affairs seeks a Social Worker (Program Coordinator) to provide care coordination and outreach to justice-involved Veterans. You will engage with correctional institutions, present VA program goals, and connect veterans to VA services and community resources.

You will coordinate overall care during transition into VA services, advocate for veterans, and liaise with VA and non-VA agencies. Strong licensure and program coordination skills are required.

Qualifications

• Must have master's degree in social work from CSWE-accredited school.
• Licensed or certified to independently practice social work at the master's level.
• US citizenship and English language proficiency required.
• Grandfathering provision may apply to current VA employees in this occupation.
• GS-12 level requires licensure/certification and demonstrated program coordination abilities.

Responsibilities

• Provide coordination of care and outreach to justice-involved Veterans.
• Initiate contact with jail administrators and establish working relationships.
• Provide information on VA services and community resources.
• Coordinate care with VA programs and community resources.
• Advocate for veterans and link to VA Medical Center and regional offices.
• Assist veterans with legal matters and educate the judicial system on VA programs.
• Perform other related duties as assigned.
