Job summary

The University of North Carolina at Chapel Hill seeks a Care Manager for the Center for Excellence in Community Mental Health (CECMH). You will support adults with serious mental illness by coordinating psychiatry, therapy, and care-team activities to develop individualized plans.

You will engage clients with frequent contact, monitor progress, and assist with transitions of care across community and clinical settings. Travel to community sites is required with reliable transportation.

Qualifications

  • Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment.
  • Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.

Position Summary

This position will serve as a Care Manager for the Center for Excellence in Community Mental Health ( CECMH ). The care management team at the CECMH offers robust and effective service models to address the comprehensive health needs of eligible beneficiaries through collaboration with insurance providers, UNC providers, and community partners.

This position will provide care management services to an assigned panel of adults with serious mental illness and other behavioral health needs. The Care Manager will engage with assigned beneficiaries through frequent contact based on client need and acuity and will be responsible for ensuring referrals to psychiatry/medication management, therapy, participation in care team huddles, and development of individualized care plans. The Care Manager will also be responsible for monitoring care and ensuring timely hospital follow-up appointments and transitions of care.

Due to the nature of this role, the Care Manager is expected to provide services in community-based settings and conduct field visits as needed. Reliable transportation is required to successfully perform the essential functions of the position.

This position will complete required trainings and learning activities through UNC CECMH on topics including HIPAA /confidentiality, proficient use of the electronic medical record, corporate compliance and ethics, cultural competence, client rights, workplace safety, and other areas relevant to the population served. In collaboration with the Care Management Program Supervisor, this position will maintain current knowledge of the care management model, policies, procedures, and expectations as disseminated through relevant partners.

Minimum Education and Experience Requirements

Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.

Management Preferences

  • Work experience with the population served (adults with serious, persistent mental illness).
  • Experience in case management or care coordination.
  • Knowledge of service systems and community resources.
