Position Summary

The Social Work Therapist is responsible for developing clinical formulations based on behavioral and psychodynamic theory, and applying treatment approaches rooted in a broad understanding of dynamics that influence behavior and change. The therapist formulates individualized, age‑specific treatment recommendations and measurable goals with the patient, and when appropriate, the patient’s family or other chosen persons. Services are provided in individual, couples, family, and group settings, and the therapist coordinates and executes discharge planning.

Qualifications
  • Minimum of a master’s degree in social work or psychology from an accredited university and at least five years of postgraduate professional, relevant experience.
  • Licensed in the State of Michigan and adhere to the code of ethics for the area of licensure.
  • Additional training in specialty areas such as chemical dependency, cognitive behavioral therapy, and/or dialectical behavioral therapy is highly desirable.
Essential Functions and Responsibilities
  • Maintains an assigned caseload in accordance with department standards.
  • Seeks consultation and supervision appropriately during patient assessment and throughout treatment.
  • Participates in supervision, case reviews, and team meetings, presenting patient cases and contributing to discussions.
  • Adjusts patient treatment plans when problems persist and recognizes when discharge or transfer of care is in the patient’s best interest.
  • Initiates discharge planning on the day of admission in collaboration with the treatment team and the patient, and when appropriate, the patient’s family.
  • Treats all patients, families, co‑workers, and visitors with respect, mindful of individual differences, cultural and ethnic diversity.
  • Completes patient‑related documentation in accordance with department and hospital policy, timely and focused on relevant issues.
  • Maintains good rapport and cooperative relationships, approaching conflict constructively and helping identify problems and solutions.
  • Maintains confidentiality of information about patients, physicians, employees, and visitors to St. Joseph Mercy Chelsea/Saint Joseph Mercy Health System.
  • Assumes responsibility for performing job duties safely, reporting preventable hazards and unsafe practices to management.
  • Engages in self‑directed learning and participates in ongoing professional development, completing all required competencies.
  • Behaves in accordance with the Mission, Vision and Values of Trinity Health.
  • Provides quality patient care by considering age‑specific, developmental, and cultural needs through competent clinical practice.
  • Identifies and assists in resolving problems related to safety, sanitation, and maintenance of the environment.
  • Demonstrates capacity to identify and manage high‑risk situations where patients may be harmful to themselves or others.
  • Understands legal issues affecting treatment, including child custody, divorce laws, child/adult abuse, duty to warn, recipient rights policies, alternative treatment orders, and the commitment process for inpatient hospitalization.

  • Maintains a patient caseload in accordance with department and/or program policy.
  • Performs other duties as assigned.

Physical Requirements and Work Environment
  • Sitting, walking, standing, bending, squatting, climbing, kneeling, twisting, lifting, and pushing tasks as described by frequency guidelines.
  • Lifting up to 35 lbs. (over 35 lbs. with mechanical assistance).
  • Exposure to fumes, chemicals, vibrations, humidity, temperature, dust, noise, and other environmental hazards.
  • Routine work with PCs and telephones; visual and auditory requirements as needed.
  • Work in environments that may involve moving mechanical parts, high or precarious places, and outside weather conditions.
  • Managing combative patients, traumatic situations involving psychiatric, dismembered, or deceased patients.
  • Blood/fluid exposure risk requiring appropriate safety categories.
Scheduling Expectations
  • Flexible hours, on‑call duty, all shifts, overtime, working holidays and/or weekends, and release time as required by department needs.
