Position Summary:

The Social Work Case Manager oversees all parenting functions within the assigned program, as outlined in program manuals. Responsibilities include working with DCYF and attorneys for clients involved in custody matters, creating, executing, and documenting personalized parenting plans based on assessed parent and child requirements, delivering parent education, and fostering a conducive environment for parenting in collaboration with the Child Care Program Director, Sr. Director of Operations, Clinical Supervisor, and Outpatient Services & Quality Director.

  • Effectively navigate conflicts with other services, both internal and external to the agency. This includes completing intakes, assessing the mother/child relationship, and devising parenting plans.
  • Offer comprehensive case management for residents, encompassing plan development, individual and group education, observation, and feedback.
  • Cultivate and maintain a safe, healthy, and nurturing learning environment for parents and children.
  • Engage in professional collaboration with clients, social workers, legal professionals, staff, and pertinent agencies involved in the well-being of patients and children.

Education:

  • Bachelor’s Degree in social work or similar field required.
  • Master’s Degree in counseling, family therapy, or social work preferred.

Experience:

  • Previous recent applicable professional experience required specifically with DCYF/CPS.
  • Experience working with women and children with at-risk situations preferred.
  • Knowledgeable in alcoholism, drug addiction, and ASAM criteria.
  • Knowledgeable in long-term rehabilitative process, including awareness of the care, post-treatment crisis, relapse, and new problems arising from need for medical sobriety etc.

Licenses/Certificates/Registration:

  • Valid drivers license and vehicle insurance and 5 years licensed driving experience required

WORKING CONDITIONS

  • Ability to understand, remember, and apply oral and written instructions or other information.
