Job summary

Hospice of Baton Rouge seeks a PRN Social Support Navigator to join our Palliative Care team. You will support patients and families navigating care options, coordinate with Social Workers, and help access community resources.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or health sciences.
  • Experience in healthcare or social services; case coordination preferred.
  • Experience with patients and families facing serious or chronic illness preferred.
  • Experience in hospital, oncology, or hospice setting strongly preferred.

Responsibilities

  • Assist Palliative Care Social Workers in coordinating patient and family support needs.
  • Make visits to patients’ homes or partner facilities to liaise between patients, families, and healthcare providers.
  • Provide education to patients and families regarding palliative care and hospice services.
  • Assist with obtaining appropriate consents from the appropriate parties.
  • Assist with nursing home placement or other care transitions as needed.
  • Connect patients and families with community resources, benefits, and support services.
  • Help identify and address psychosocial, emotional, or practical needs of patients and caregivers.
  • Maintain accurate and timely documentation of interactions and referrals.
  • Participate in interdisciplinary team discussions and care coordination meetings.
  • Uphold confidentiality, professionalism, and compassionate communication at all times.
  • Perform other related duties as assigned by the Palliative Care Director or designee

POSITION SUMMARY

The Hospice of Baton Rouge is seeking a compassionate and skilled PRN Social Support Navigator to join our dedicated Palliative Care team. This person will provide support to patients and families receiving palliative care, helping them navigate care options and access needed resources. The Social Support Navigator will assist Palliative Care Social Workers with coordination, communication, and education to ensure patients experience a smooth and supportive continuum of care.

JOB RESPONSIBILITIES
  • Assist Palliative Care Social Workers in coordinating patient and family support needs.

  • Provide education to patients and families regarding palliative care and hospice services

QualificationsEducation
  • Bachelor’s degree from an accredited college or university required, preferably in Social Work, Psychology, Sociology, Human Services, or a health sciences-related field.
Work Experience
  • Previous experience in healthcare, social services, patient support, or case coordination preferred.
  • Experience working with patients and families facing serious or chronic illness is highly desirable.
  • Experience in an acute care, oncology, or hospice setting strongly preferred.
Special Skills
  • Strong interpersonal and communication skills.
  • Ability to demonstrate compassion, empathy, and emotional resilience.
  • Capable of handling sensitive information and conversations professionally.
  • Excellent organizational and time-management abilities
  • Familiarity with community health resources, senior care, or healthcare systems preferred.
Other Requirements
  • Strong communication skills with patients, families, physicians, facilities, and staff.
  • Installation of company communication application on cellular device.
  • Must have reliable transportation, a current Louisiana driver’s license, and proof of liability insurance that meets state requirements for occasional travel to patient facilities or partner organizations.
  • Must maintain confidentiality in accordance with HIPAA and organizational policies.
