Job summary

ALIYA Healthcare Consulting in Oak Lawn, IL seeks a Social Services Discharge Planner to coordinate the transition of residents back to the community and oversee the Social Services Department operations. You will review policies, conduct psychosocial assessments, and collaborate with an interdisciplinary team to protect resident rights and address psychosocial needs.

Qualifications

• Bachelor's degree in Social Work or related field is required.
• 2+ years of experience in social work or a related field.
• Strong understanding of social work principles, theories, and practices.
• Excellent communication, interpersonal, and organizational skills.

Responsibilities

• Review facility policies and procedures to ensure compliance with state and federal regulations.
• Participate in setting policies concerning resident care and quality of life.
• Develop facility social work policies and participate in quality assurance interdisciplinary team meetings.
• Document progress in meeting the psychosocial needs of residents and work with the interdisciplinary team to promote and protect resident rights.
• Complete social histories and psychosocial assessments for each resident, identifying their social, emotional, and psychological needs.
• Provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility.
• Provide support and education to residents and their families/significant others, referring them to appropriate social service agencies when necessary.
• Coordinate the resident discharge planning process and make referrals for further services.
• Provide clinical interventions to address catastrophic events that occur during a resident's stay.

As a Social Services Discharge Planner, you will play a vital role in ensuring the smooth transition of our residents back into the community.
