Harmony Palos Heights is seeking a Social Services Director for our Skilled Nursing Facility. You will lead social work services, coordinate resident discharges, and participate in care conferences to ensure residents’ social and emotional needs are met.

The ideal candidate holds a Bachelor’s in Social Work (or similar) with at least two years in a healthcare social work role; a graduate degree and five years of related experience are preferred. Equal opportunity employer.

• Bachelor's degree in Social Work or similar field.
• Minimum two years of experience in a Social Worker role in healthcare setting.
• Social Services Director experience in a Skilled Nursing Facility preferred.
• Graduate degree in Social Work or similar field and five years of related experience preferable.
