Job summary

Merriam Gardens Healthcare and Rehab Center seeks a licensed Social Worker to counsel assigned clients and their families. The role serves as a resource to other staff and participates in interdisciplinary rounds, discharge planning, and follow-up care.

The candidate will coordinate schedules with therapy and nursing teams, maintain thorough records, and participate in ongoing professional development as directed by supervision.

Qualifications

• Must hold a Degree in Social Work and applicable license/certification.
• Minimum of two years experience in a health care setting, preferably IRF.

Job description

SUMMARY:

The Social Worker is responsible for the counseling of assigned clients as well as their family members. The Social Worker acts as a resource person to other program staff and to the facility in the area of counseling, family dynamics/interactions. The Staff Social Worker is under the direct supervision of the Administrator.

DUTIES PERFORMED:

  • Conducts psychosocial history to determine pre-injury issues which need to be addressed.
  
  
  • Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.
  
  
  • Provides counseling to clients, families and/or significant others.
  
  
  • Develops and implements family and children support groups as needed.
  
  
  • Schedules, arranges and moderates the program for family education group.
  
  
  • Participates in team rounds and family conferences as scheduled through case manger.
  
  
  • Maintains records and prepare reports according to facility procedures.
  
  
  • Participates in and documents internal and external professional educational activities to enhance personal and professional growth.
  
  
  • Performs public relations task as directed by supervisor.
  
  
  • Coordinates client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist and other disciplines as needed.
  
  
  • Is an integral part of the interdisciplinary team.
  
  
  • Assists in patient discharge planning.
  
  
  • Conducts post discharge follow-up as needed.
  
  
  • Any other tasks as assigned by the immediate supervisor.

  • Formal Education/Formal Training: Must hold a Degree in Social Work and applicable license/certification.
