Job summary

Avante Leesburg Skilled Nursing and Rehabilitation Center in Leesburg, FL is seeking a Social Services Director to act as liaison for residents’ social interests and to plan, organize, develop and direct the Social Services Department in accordance with federal and state standards.

You will ensure that the medically related psycho-social needs of residents are met, work with administration, and collaborate with families.

Qualifications

• Bachelor's degree in Social Work or Human Services required.
• Minimum of 3 years’ experience in Long Term Care.
• At least 1 year of supervised Social Work experience in a Health Care setting.

Responsibilities

• Act as liaison and representative of residents' social interests; plan, organize, develop and direct the Social Services Department in accordance with federal/state standards.
• Ensure medically related psycho-social needs of residents are met and maintained on an individual basis.
• Collaborate with administration, residents, and families to ensure quality social services.

What we are looking for:

• Positive attitude toward the elderly.
• Must be a team player.
• Must have the ability to work with all levels of management.
• Must have strong problem-solving skills.
• Must have excellent written and verbal communication skills.
• Knowledge of Outlook, Excel and Word.
• A Bachelor’s Degree in Social Work or in Human Services but not limited to Sociology, Psychology or Rehabilitation Counseling.
• Minimum of 3 years’ experience in Long Term Care.
• At least 1 year of supervised Social Work experience in a Health Care setting working directly with individuals.
