Job summary

Hilltop at Blue River in Kansas City, MO is seeking a compassionate Social Services Director to oversee psychosocial care in our skilled nursing facility. You will support residents' emotional, social, and mental well-being, ensure smooth transitions, and advocate for quality of life.

In this leadership role, you will conduct psychosocial assessments, develop care plans, coordinate discharge planning, document in accordance with guidelines, and educate staff to maintain compliance and survey

Qualifications

• Bachelor’s degree in Social Work or a human services-related field (such as Psychology, Sociology, Special Education, or Rehabilitation Counseling).
• Minimum of one year of supervised social work experience in a healthcare setting working with elderly populations.
• Experience in a Skilled Nursing Facility (SNF) preferred.

Responsibilities

• Complete psychosocial assessments and develop individualized care plans.
• Maintain accurate and timely documentation in compliance with state and federal guidelines.
• Provide emotional support and crisis intervention for residents and families.
• Coordinate discharge planning and referrals to community services.
• Advocate for residents’ rights and collaborate with the interdisciplinary care team.
• Educate staff and support compliance and survey readiness.
