Job summary

Cypress Skilled Nursing is seeking a dedicated Social Services Director to join our team of professionals. The role focuses on assessing residents’ social and emotional needs and coordinating care plans, MDS, and psychosocial assessments.

The candidate will participate in admission planning, transfers, and interdisciplinary care plan meetings, ensuring a compassionate resident-centered approach and effective monthly residents’ council meetings.

Qualifications

• A Bachelor's Degree in Social Work, Phycology, Sociology, or Human Relations from an approved school is required.
• Must have a minimum of two (2) years of experience in a supervisory capacity at a hospital, skilled nursing facility, or other related medical facility.
• Must have knowledge of state and federal regulations.
• Must have strong verbal and written communication skills.
