Job summary

St. Gabriel's Community is seeking a Social Services Designee to coordinate the interdisciplinary care plan for residents from admission to discharge. You will act as liaison between residents, families and healthcare staff to ensure timely, effective psychosocial support.

You will develop and maintain community resources, advocate for residents and families, and promote a comforting, empowering culture across our nonprofit senior care system in the upper Midwest.

Qualifications

• Related coursework in social work or psychology
• Hospital or nursing home experience, preferred
• Able to read, write and communicate in English
