Job summary

National Health Foundation in Ventura, CA, is seeking a full-time Social Services Coordinator to provide post-hospital care for guests experiencing homelessness transitioning from acute care. You will perform needs assessments, coordinate mental health and housing services, and support discharge planning.

This role requires a Bachelor’s degree with 3+ years in healthcare or social services, strong communication, and excellent organizational skills.

Qualifications

• Bachelor’s Degree required.
• Master’s in Social Work preferred.
• Minimum of three years relevant healthcare/social services experience.
• Excellent communication skills (oral and written).
• Strong time management and organizational abilities.
• Proficiency with MS Word, Excel, Outlook, and PowerPoint.
• Reliable transportation.
• TB test required.
• Fingerprint live scan/background check required.

Responsibilities

• Perform Needs Assessments and create Care Plans.
• Coordinate services for mental health, housing, and related needs.
• Maintain CRM/proprietary database and document compliance (CES/HMIS).
• Provide emergency referrals and warm handoffs as appropriate.
• Attend care coordination meetings with nursing and other teams.
• Support guest interventions and crisis de-escalation when needed.
• Engage with guests daily to build rapport and trust.
