Job summary

Monroeville Post Acute is a 131-bed post-acute care facility offering compassionate care at 885 MacBeth Dr., Monroeville. We seek dedicated team members who put residents first and strive for excellence in action daily.

Successful candidates will have Social Services experience and strong communication and documentation skills. Skilled nursing experience and PCC exposure are a plus as you collaborate with residents, families, and the care team to ensure a solid discharge plan.

Successful candidates will have the following:

• Social Services experience
• Skilled nursing experience, preferred

Your day to day:

You'll collaborate with residents, their families, and their care team to ensure the patient's needs are being met and that there is a solid discharge plan as needed. You'll work under supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
