Job summary

Aviata Health Group in Kissimmee, FL, is seeking a Social Services Assistant to support residents and families, coordinate social work services, and assist with discharge planning in a compassionate, resident-centered care environment.

You will document evaluations, communicate with facility staff and families, and help ensure regulatory compliance while contributing to a culture of care and continuous improvement across the team.

Qualifications

• High school diploma or equivalent required.
• 1 year of long-term care experience preferred.

Responsibilities

• Conduct and document social services evaluations identifying resident problems/needs.
• Provide or arrange social work services as indicated by resident/family needs.

• Meet with facility staff to discuss resident issues and plan care.
• Assist with discharge planning.
• Ensure compliance with regulatory standards and internal policies.
• Contribute to creating and implementing new ideas to improve systems.

About the Role

We are searching for a Social Services Assistant to join our community that is resident and family focused, a team builder, and excited about the opportunity to assist in building a facility culture. The primary purpose of your job position is to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities

• Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Executive Director/LNHA.
• Conduct and document a social services evaluation, including identification of resident problems/needs.
• Provide/arrange for social work services as indicated by resident/family needs.
• Assist resident, as needed, with financial planning.
• Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment.
• Meet with the appropriate facility staff concerning resident issues.
• Act in compliance with all corporate, state, federal, and other regulatory standards.
• Assist with discharge planning.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Minimum Qualifications

• Must possess a high school diploma or equivalent.
• One (1) year experience in long-term care preferred.
• Must demonstrate the ability to assess resident needs and coordinate the discharge planning process.
