Citrus Heights Community Healthcare, LLC is seeking a Social Worker for our Post-Acute program. The role requires a Bachelor’s in Social Work or related field and 2 years of supervised social work experience in a health care setting.
The position offers a pay range of $30.00–$35.00 per hour based on experience and Monday through Friday scheduling. Responsibilities include coordinating care with residents and families, developing care plans, and connecting with community resources to meet needs

This job has no supervisory responsibilities.
Education and/or Experience: Bachelor’s Degree in Social Work or in Human Services and 2 years of supervised social work experience in a health care setting working directly with individuals. MSW preferred.
