Job summary

Harmonia Collaborative Care is seeking a compassionate, organized Social Care Navigator to help community members connect with services and resources that support their health and well-being.

You will complete social care screenings, connect individuals with community resources, make referrals and follow up, verify eligibility, document actions, and communicate with clients, providers and partners to help people navigate barriers and access needed support.

Qualifications

• High school diploma or equivalent is required or accepted.
• 1–2 years of experience in care coordination, healthcare, community services, public health, nonprofit services or a related field.
• Strong communication and organizational skills are essential.
• Comfortable working with individuals from a variety of backgrounds.
• Community Health Worker certification is welcome but not required if other qualifications are met.
