Job description

The SNHP Lead Case Manager is responsible for providing comprehensive case management services to individuals experiencing homelessness, supporting their transition into stable housing through the Special Needs Housing Program. This role combines direct client service with leadership responsibilities, including team support, program coordination, and collaboration with internal and external partners. The Lead Case Manager ensures high-quality service delivery, compliance with program requirements, and alignment with organizational goals to improve housing stability and client outcomes.

Essential Duties and Responsibilities:

Leadership and Organizational Support:

  • Provide operational supervision and guidance to team members in the absence of the Program Manager.
  • Support team performance by offering training, mentorship, and assistance with complex cases.
  • Collaborate with Program Manager on program implementation, reporting, and special projects.
  • Contribute to team meetings, case conferencing, and continuous quality improvement efforts.
  • Ensure alignment with HOPE Atlanta’s mission, policies, and service delivery standards.

Client Engagement and Management:

  • Direct and enhance the team’s efforts to engage with and build trust among homeless
  • individuals, with a focus on diverse and vulnerable groups.
  • Conduct comprehensive client intakes, including documentation collection, assessments, and housing plan development.
  • Identify and address barriers to housing placement and stability.
  • Develop and monitor individualized housing plans to support placement into mid-term or permanent housing.
  • Maintain regular contact with clients (minimum monthly) to track progress and provide ongoing support.
  • Provide crisis intervention and support to clients experiencing urgent needs.

Service Coordination and Referrals:

  • Ensure effective connections for clients with relevant services and benefits, including healthcare, housing, and emergency services.
  • Collaborate with HOPWA programs and community partners to ensure coordinated service delivery.
  • Assist clients in navigating housing applications and securing safe, affordable housing options.
  • Facilitate communication between clients, housing providers, and partner agencies to resolve concerns.

Documentation, Reporting, and Compliance:

  • Maintain accurate and up-to-date client records in HMIS and other required databases.
  • Ensure all client files meet program and compliance standards.
  • Track and report client outcomes, program data, and performance metrics.
  • Review cases regularly with management and participate in audits or file reviews.
  • Maintain confidentiality and adhere to all program regulations and policies.

Collaboration and Community Engagement:

  • Foster partnerships and collaborations to prioritize outreach efforts and integrate clients into the Coordinated Entry system.
  • Lead the team in community engagement activities, including the annual Atlanta CoC Point in Time count.
  • Participate in multidisciplinary team meetings and case conferencing.
  • Support broader organizational initiatives and community efforts to address homelessness.

Additional Duties and Responsibilities:

  • Adapt to additional duties as required, focusing on client welfare and service delivery excellence.
  • Ensure team readiness to provide services beyond conventional hours, as necessary.
  • Assist with data collection, reporting, and program evaluations.
  • Support team efforts during high-demand periods or staffing gaps.

Knowledge, Skills, and Abilities:

Advanced Knowledge:

  • Homelessness issues, local resources, legal/ethical standards, and mental health, substance use, and at-risk populations.
  • Understanding of HOPWA regulations and housing program requirements.

Leadership Skills:

  • Exceptional communication, crisis intervention, documentation, organizational, and problem-solving skills.

Technical Skills:

  • Proficiency in MS Office Suite and HMIS database systems.
  • Experience with Salesforce Voucher entry.

Abilities:

  • Strong adaptability, interpersonal sensitivity, teamwork, initiative, and cultural awareness.

Qualifications

  • Bachelor’s degree in social work, psychology, public health, or a related field with at least one year of experience in a leadership role working with the homeless population, or an equivalent combination of education and experience.
  • Demonstrated experience working with homeless or vulnerable populations.
  • Minimum of three (3) years of experience in social services or case management.
  • Proven ability to manage teams in dynamic environments.
  • Valid driver’s license, reliable transportation, and a good driving record required.
