Job summary

Creekstone Health and Wellness in Kissimmee, FL is seeking a Physical Therapist (PT) to evaluate, plan, and implement individualized therapy programs for residents in a Skilled Nursing Facility. The role focuses on improving mobility, independence, safety, and quality of life while meeting Medicare and state regulations.

You will work with an interdisciplinary team, educate residents and families, document accurately in EMR, supervise PTAs as allowed, and help meet facility performance goals.

Qualifications

• DPT degree from an accredited program.
• Current Florida Physical Therapist License in good standing.
• CPR/BLS certification preferred; Florida licensure is required for practicing PTs within the state.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Assess resident mobility, strength, balance, gait, range of motion, pain levels, and functional abilities.
• Provide skilled therapeutic interventions, including therapeutic exercise, gait training, balance training, transfer training, neuromuscular re-education, and manual therapy.
• Establish realistic short-term and long-term rehabilitation goals.
• Monitor resident progress and modify treatment plans as appropriate.
• Educate residents, families, and caregivers regarding treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate resident care.
• Complete accurate and timely documentation in accordance with Medicare, Medicaid, Florida regulations, and facility policies.
• Participate in care plan meetings and interdisciplinary team conferences.
• Ensure compliance with all infection control, safety, and quality assurance standards.
• Supervise Physical Therapist Assistants (PTAs) and rehabilitation aides as permitted by state regulations.
• Maintain productivity standards and support facility performance goals.
