Centra Healthcare Solutions is seeking an experienced Physical Therapist (PT) licensed or in the process of obtaining WI licensure to work in SNF/LTC settings on a contract basis. This role requires 1 year of PT experience in SNF/LTC and WI licensure, with on-site 5x8 days schedule.

Qualifications

• 1 year of recent experience as a Physical Therapist (PT).

• Collaborate with care team to implement treatment plans.
• Maintain patient progress notes and documentation.
