Job summary

Qualifications

• Must have a valid professional SLP license in the state of practice.
• Graduate of ASHA accredited school; if foreign-trained, equivalency to U.S. entry level education must be established by ASHA-approved credential evaluation.

Responsibilities

• Complete evaluations, assessments, and treatments according to the care plan.
• Complete documentation and records in a timely and accurate manner.
• Follow policies and procedures related to the scope of practice.
• Recognize priorities in patient care and exercise sound judgment.
• Attend required orientations, in-services, and trainings on time.
• Perform other related duties as directed by supervisor.
