Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team. The incumbent will provide direct patient care, assess rehabilitation needs, and develop treatment plans under physician direction.

Qualifications include a degree in Speech Language Pathology, Tennessee licensure, and experience with geriatric populations. Collaboration with interdisciplinary teams and strong documentation skills are essential.

Qualifications

• Graduate from an accredited Speech Language Pathology program.
• Valid TN state license and registration for state TN.
• Experience in long-term/geriatric care preferred.
• Proficient in medical record documentation.
• Collaborates with disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment goals per physician guidance.
• Document patient condition and educate patients and families.
• Maintain confidentiality and HIPAA compliance.
• Perform duties as assigned by Director of Rehabilitation.
