Job summary

Mountain Ridge Rehabilitation And Healthcare Center, located in Black Mountain, NC, is hiring a Speech Therapist (SLP). Accepting CFY candidates. Join a growing team of dedicated caregivers who are valued and appreciated.

You will provide evaluations, develop treatment plans, document progress, and collaborate with an interdisciplinary team to support resident communication and quality of life.

Qualifications

• Must have MA or MS in communication disorders.
• Must be ASHA and state board recognized.
• Must hold current license/registration in the state.
• CFY completion required.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities, therapeutic interventions, clinical and technical guidelines.
• Ensure MD orders are obtained for evaluations, treatments, recertification's and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
• Document patient’s progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format in timely manner.
• Document patient’s discharge and in-service as indicated.
• Document any caregiver or discharge in-services in approved format as indicated.
• Complete any and all other documentation required by Facility.

Education

MA/MS in communication disorders

ASHA/state board recognition

Current license/registration in state
