SUMMARY: Under direct or indirect supervision of Charge Nurse or Department Leader, The SLH Unit Clerk contributes to patient care by performing clerical tasks directly and indirectly related to patient care and unit functioning. They are responsible and accountable for tasks delegated. Performs related duties as required.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

1. Transcribes all physician orders according to hospital policy and procedure. Communicates pertinent physician orders to the Registered Nurse and/or Charge Nurse as per unit policy/guidelines.

2. Prepares and assembles charts for admission according to hospital policy and procedure. Ensures proper chart maintenance and chart flow. Maintains patient confidentiality. Obtains medical records. Tears down charts. Collects Emergency Dept. Statistics. Checks charts for completion. Ensures all patient records are stamped appropriately. Sorts and places patient's charts in appropriate area during the course of patient care. Places patient labels on all medical record forms. Copies charts, records and assists in preparing transfer forms.

3. Notifies RN and Admitting Dept. of patient's arrival. Notifies patient's private physician when appropriate. Listens to patient/family concerns, refers to RN/MD as appropriate. Dispenses MD lists, referrals as directed. Updates Physician Referral lists.

4. Monitors MED/NET and keeps Charge RN informed regarding EMS communications. Routes reports to other dept. Communicates with other hospital departments.

5. Maintains PAR Level of forms, referral lists, D/C Instructions, Core Measure Packets. Picks up and distributes mail from mail room. Maintains ED Log Book. Processes Culture reports, logs on EDMD log.

6. Completes patient level and procedures charges with accuracy. Ensures all emergency dept. charges for services and supplies are entered in the computer. Completion of discharge duties/info (Rx industrial patients to pharmacy, completed work forms, referral appointments made, etc) prior to discharge of patient.

MINIMUM QUALIFICATIONS: Education: High School Graduate or GED. Minimum Experience: One year previous unit secretary experience. Required Licenses/Certifications: BLS Basic Life Support Certification - issued by AHA-American Heart Association.
