Job summary

United Cerebral Palsy Associations Inc. seeks a dedicated professional to provide independent living skills training on a one-on-one basis under supervision.

Responsibilities include facilitating training, supporting personal care, mobility and communication needs, and documenting services for individuals with disabilities. Qualifications

• High school diploma or GED; preference for experience in day or residential programs serving persons with disabilities.
• Must read, write and understand English; strong oral and written communication; follow verbal and written instructions from supervisor.
• Must have working knowledge of personal computers.
• Must have CDL and driving record; transport responsibilities; proof of ongoing auto insurance on personal vehicle.
• Must maintain First Aid/CPR certificates and meet California Community Care Licensing standards.

Responsibilities

• Promote and adhere to the Supported Living Principles: A Home of One's Own, Choice and Self-Directed, Relationships, Community Membership, Flexible, Tailored Services and Supports.
• Assist in developing and implementing: supports service activities and independent living training, management plans, and supported living assessments.
• Carry out specific skills training plans for participants as instructed.
• Assist participants in all personal care needs (toilet, feeding, hygiene).
• Assist participants with mobility and communication needs.
• Develop natural and community support.
• Assist in medical appointments and health needs.
• Maintain detailed documentation of services provided.
• Assist with preparation of reports on participants.
• Participate in regular staff meetings.
• Maintain appropriate communication with supervisor(s).
• Other duties as assigned.

SUMMARY

Under general supervision of the Program Manger is responsible for independent living skills training on a one-on-one basis.

QUALIFICATIONS

Education/Experience

High school diploma or GED with a preference give to persons who have experience working in day or residential programs serving persons with disabilities.

Language Ability

Must possess ability to read, write and understand English. Strong oral and written communication skills are a must. Must be able to understand and follow verbal and written instructions from supervisor.

Math Ability

Knowledge and ability to work with basic mathematical concepts.

Computer Skills

Must have working knowledge of personal computers. (Highly desirable)

Driving Skills

Must possess a valid CDL and have clean driving record as well as operate a vehicle in good working order for employee transportation. Must have proof of ongoing auto insurance on personal vehicle.
