Job summary

United Cerebral Palsy Associations Inc is seeking a dedicated individual to provide independent living skills training on a one-on-one basis under the supervision of the Program Manager. The role involves assisting participants in daily living activities, mobility, communication, and community integration.

Responsibilities include developing training plans, documenting services, participating in staff meetings, and ensuring safety through adherence to First Aid/ CPR and California licensing

Qualifications

  • High school diploma or GED; experience in day/residential programs preferred.
  • Ability to read, write and understand English; strong oral and written communication.
  • Knowledge of basic mathematical concepts.
  • Working knowledge of personal computers.
  • Valid CDL with clean driving record; must have vehicle insurance for employee transportation.
  • First Aid/CPR certs and meeting California Community Care Licensing standards may be required.

Responsibilities

  • Promote and adhere to the Supported Living Principles.
  • Develop and implement support service activities and independent living training.
  • Assist with management plans and supported living assessments.
  • Carry out skills training plans for participants and assist with personal care needs.
  • Maintain detailed documentation of services provided and participate in staff meetings.
  • Maintain communication with supervisor(s) and handle other duties as assigned.

SUMMARY

Under general supervision of the Program Manger is responsible for independent living skills training on a one-on-one basis.

QUALIFICATIONS
Education/Experience

High school diploma or GED with a preference give to persons who have experience working in day or residential programs serving persons with disabilities.

Language Ability

Must possess ability to read, write and understand English. Strong oral and written communication skills are a must. Must be able to understand and follow verbal and written instructions from supervisor.

Math Ability

Knowledge and ability to work with basic mathematical concepts.

Computer Skills

Must have working knowledge of personal computers. (Highly desirable)

Driving Skills

Must possess a valid CDL and have clean driving record as well as operate a vehicle in good working order for employee transportation. Must have proof of ongoing auto insurance on personal vehicle.

  • Must maintain First Aid/CPR certificates and be able to meet California Community Care Licensing personnel standards

Assist in developing and implementing:

  1. Support service activities and
  2. Independent living training,
  3. Management plans
  4. Supported living assessments
  • Carry out specific skills training plans for participants as instructed
  • Assists participants in all personal care needs (toilet needs, feeding and hygiene)
  • Assist participants with their mobility and communication needs
  • Develop natural and community support
  • Assist in medical appointments and health needs
  • Maintain detailed documentation of services provided
  • Assist with preparation of reports on participants
  • Participate in regular staff meetings
  • Maintain appropriate communication with supervisor(s)
  • Other duties as assigned
