Job summary

Centerstone is seeking a dedicated professional to teach, guide, and support individuals with serious mental illness or substance use disorders, helping them overcome barriers in daily living and recovery. You will coach clients toward self-advocacy, problem solving, and budgeting while working across home and community settings.

Responsibilities include team-based treatment planning, timely clinical documentation, and coordination with multiple systems to support client progress and recovery

Qualifications

  • Education level: Bachelor’s degree preferred; Associate degree or two years of related experience may substitute.
  • Minimum two years professional experience with adults/youth and families in behavioral health or related setting preferred.
  • Obtain and maintain CANS/ANSA certification.

Responsibilities

  • Communicate client status with interdisciplinary team and referral sources.
  • Participate in team meetings to discuss treatment plans.
  • Complete clinical documentation accurate and timely per organizational standards.
  • Complete authorizations paperwork for clients (ASAMs, Care Plans, CANS/ANSA) and related assessments.
  • Provide individual and/or group interventions in home, community or office settings.
  • Collaborate with community systems (criminal justice, child welfare, hospital, education).

JOB DESCRIPTION: SUMMARY OF POSITION

Teaches, guides, instructs and supports persons with serious mental illness and/or substance use disorders to address barriers in any life domain, associated with their recovery. Identifies and meets the client where they are and coaches them to success. Teaches skills such as: self-advocacy, negotiation, problem solving, goal setting, symptom management, recovery capital, daily living (i.e. housekeeping, cooking, cleaning, and parenting), communication, coping, financial/budgeting and money management, benefits management, vocational, identifying triggers to prevent relapse, community integration, and social/recreation/leisure skills.

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Communicates client status with interdisciplinary team and referral sources.
  • Participate in team meetings for the discussion formulation of the treatment plan.
  • Complete clinical documentation that is accurate, meets organizational standards in composition and content, and is adequate for communication about client’s treatment and progress.
  • Completes authorizations paperwork for clients including ASAMs, Care Plans, CANS/ANSA's and all documentation/assessments necessary for treatment specific to the program.
  • Complete clinical documentation within 24 hours of the service provided.
  • Uses structured, standardized interventions and resources for teaching to assist clients in overcoming barriers to recovery.
  • Provides individual and/or group interventions in the home, community or office setting.
  • Works within various community systems to collaborate on behalf of, and with, the client. Includes the criminal justice system, the child welfare system, hospital/medical systems, educational, etc.
  • Assesses and updates person-centered plan with the client and collaterals and monitors progress of the plan.
  • Ensures client’s clinical record is maintained and up to date at all times.
  • Clinical documentation is accurate, meets organizations standards in composition and content, and is adequate for communication about client’s treatment and progress.
  • Submits reports to referral sources on clients participation and progress in treatment, where authorized.
  • Understand how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
  • Perform all required job duties and responsibilities in accordance to CCBHC Standards.

QUALIFICATIONS
  • Education Level Bachelor’s level preferred, Associate's degree or Two years of experience or two years of college coursework in related field may be substituted for degree requirement.
  • Years of Experience Minimum of two years professional experience working directly with adults or youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.
  • Certification/Licensure Obtain and maintain CANS/ANSA Certification.
  • Time Type: Full time
  • Pay Range: $19.80--$23.00
