Job summary

The Highlands at Owasso in Owasso, OK is seeking a skilled Physical Therapist to provide PT services in a skilled rehab setting. The role is full-time with weekday hours. Licensure in Oklahoma in good standing is required.

You will develop therapy plans, collaborate with the care team, educate patients and caregivers for home care, and maintain thorough documentation to support quality outcomes. Experience with post-acute rehab is a plus and the ability to adapt plans to evolving patient needs

Qualifications

• Current license in the State of Oklahoma in good standing.
• Strong communication and interpersonal skills, both verbal and written documentation.

Responsibilities

• Perform therapy treatments and functions for skilled nursing patients.
• Provide treatment plans that allow patients to reach their maximum potential or for successful transition to next level of care.
• Work with other therapists and nursing staff to provide cohesive quality of care.
• Educate patients and caregivers to be successful at home after discharge from the facility.
• Provide quality documentation as required.
