West Covina Healthcare Center is seeking a licensed Physical Therapist to evaluate and plan individualized therapy programs for residents in a skilled nursing setting. You will work to maximize residents' functional abilities while collaborating with physicians, nurses, and other therapists.

Ideal candidates hold a DPT or equivalent, have active licensure, and possess strong clinical and communication skills.

Qualifications

• Doctorate or master's degree in physical therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing facility or similar setting preferred.
• New graduates with strong clinical skills are encouraged to apply.

Responsabilités

• Conduct thorough evaluations of residents' physical abilities and limitations.
• Develop individualized treatment plans based on assessment findings.
• Implement physical therapy interventions to improve strength, mobility, balance, and coordination.
• Monitor and document residents' progress and adjust treatment plans as necessary.
• Work closely with other members of the healthcare team, including physicians, nurses, and occupational therapists.
• Communicate effectively with residents and their families regarding treatment plans and progress.
